1. A copy of your new Polaris 380 sales receipt.
2. The completed 380 warranty extension card.
3. This completed Certificate.
4. The main body (no hoses or pumps) of old automatic cleaner working or not.

This certificate is not redeemable with a dealer. Polaris must be in receipt of all items no later than the final date of this offer, September 30, 2003.
