Raymond- For your model Max-E-Pro the USQ1152 A.O. Smith 1 1/2 H.P Square Flange Up Rate Motor, the shaft seal I would recommend is the Shaft Seal PS-201V, 3/4" Shaft Size (PS-201V-CMS) part number 35-423-1096 and the band clamp you would need for it is the Clamp, Tank Body (v26-351) part number 5056-12A.
