It does not come with fiber optic cable. The cable is sold separately per foot at http://www.inyopools.com/fiber-optics-underwater.aspx.

Does this come with everything to just connect over fiber optic cable (does it include small cylinder aluminum piece that goes under neath head cap?

Good Morning Chuck, Yes, It comes with 2 pieces one for bottom and one for the top piece under the lens also bushing piece and pvc
