Hello David, Here is a link to the Sta-Rite Super Max Parts list. This is the correct Seal Plate - Black (355612) that fits the Sta-Rite Super Max.

I just purchased the Century 1.5 HP Round Flange 56J Up Rate Motor - B2 (Item #5212X). Will this mounting plate fit this motor and do the screws come with the part? Please advise
