The rise on these steps are roughly 11 inches per step.

It states weight is needed to ensure the Wide Mighty Step is secure. Can you please explain what is recommended and how it is attached?

You would add sand through openings in the ladder. You can take a look at the owner's manual for more in depth explanation of the assembly.

Have a vinyl pool liner. Need steps for mom to get in and out of pool. Does this item include a pad? Do I have to attach or is it used with sand?

The step pad is sold separately. There are brackets to attach to the deck or top rail of the pool and the step also requires weight to hold it in place (typically sand or pea gravel depending on the manufacturer's instructions).

If we purchased the additional weights, is it still necessary to put sand in the steps? We didn't do that & the steps keep tilting up & falling apart. Risers are falling out & the 2 bottom base pieces keep falling off.

Hi, Erin. If you use the Step Weights - AC70033, you won't have to put sand in the Side Panel.
